Sub-Acute Rehab empowers the patient and their family for a successful transition from the hospital, while maximizing their ability to participate in meaningful living.
Also referred to as Swing Bed, Sub-Acute Rehab allows patients to move their level of care from acute to rehabilitation but remain in the hospital setting. Our skilled and compassionate staff works with both the patient and their family to create individualized treatment plans based on specific needs and goals — to help individuals heal, gain strength and regain independence.
Our program is ideal for patients recovering from:
- Generalized deconditioning
- Joint replacement or other types of surgery
- Stroke, cardiac or respiratory illnesses, or other medical conditions
- Post-surgical care required such as long-term antibiotic therapy and debridement that can't be provided at home or in an outpatient setting.

Why is Sub-Acute Rehab also called Swing Bed?
Swing Bed is a government, insurance-based term that allows patients to "swing" or move from an acute level of care to a skilled rehabilitation level of care, but remain in a hospital setting.
Can Sub-Acute Rehab be covered by my insurance?
Yes. Sub-Acute Rehab is covered by Medicare as well as many private insurance plans. Please contact us to discuss your specific plan.
I've been discharged home from a hospital, but am having difficulties recovering as an outpatient. Can I still enter Sub-Acute Rehab?
Yes — as long as you have stayed in the hospital for at least three qualifying nights in a row within the last 30 days.
What's a typical day like?
Our care team will set a personalized treatment plan tailored to your individual needs and goals. You may receive physical and/or occupational therapy, aquatic therapy, IV therapy or other care. Similar to the hospital, you'll receive 24-hour nursing care that is overseen by one of our physicians. When you're not in therapy, you can spend time visiting with family and friends, participating in one of our planned activities, or simply relax.
How long can I stay?
You can remain in the program as long as you have therapy and rehabilitation goals to meet. Once these goals have been met, some patients are able to return home and some patients may need to explore additional care options. We will help by providing recommendations and alternatives for future care when necessary.
